Linkages

Linkages programs use a case management approach, and where appropriate utilise available discretionary funds to provide individually tailored packages to assist frail or at risk older people, people with moderate to severe disabilities and their carers.

Linkages programs aim to organise and coordinate community care services, to monitor and respond to changing needs and to provide support and advocacy to assist people who have complex care needs to remain living in the community.
